PROGRAM Queens West Foster Care
LOCATION: Genovese Family Life Center
161st Street
Jamaica, NY 11432
BASIC FUNCTION:
To provide comprehensive and referral services to children and their families in order to facilitate reunification and permanency for children.
RESPONSIBILITIES:
• Counsel and refer birth families for concrete supportive services.
• Coordinate the services necessary to achieve permanency through reunification or adoption.
• Attend foster parent and birth parent home and school visits, participate in meetings with community programs, attend Family Court hearings, conduct
extensive crisis intervention.
• Complete progress notes, FASP/Risk Assessments and all necessary accountability paperwork.
• Maintain up to date case records.
QUALIFICATIONS:
• BA/BS Degree in Health or Human Services required.
• Bilingual/ Spanish is a plus.
• Related work or field experience preferred.
• Strong advocacy, communication and organizational skills are necessary.
